Community Dental Services facilitates comprehensive dental health care for the long term care resident. Participating residents will benefit from the convenience of dental and hygiene services that can be safely and comfortably delivered within their long-term care facility. Many preventive, basic and maintenance dental services can be performed on location for maximum convenience to the facility and resident. Complicated procedures may be referred off site. Community Dental Services will assist the resident through this process to assure compliance with their Dentist prescribed oral health plan.

Community Dental Services knows the value of working together as a team. The multidisciplinary team includes the patient, their family, facility care givers, administrators, social workers, physicians and specialists. Maximizing the benefits of this multidisciplinary approach CDS assures a complete oral health care plan to meet the resident’s specific needs.

Community Dental Services acknowledges a complete team is paramount for a successful program. Progress reports are generated for each resident and communicated with the team regularly. Twenty-four hour access available to all team members for urgent oral health needs. Community Dental Services actively participates in applicable patient care conferences and promotes their monthly educational programs for families and all care givers.

Home Bound, Assisted and Independent Living Residents

Community Dental Services (CDS) offers dental and hygiene services to access challenged patients.
Access challenged patients may have limited mobility, are recovering from a serious illness or surgery, or are home bound due the natural maturing process of life.

CDS assists non-ambulatory residents to receive access to dental care. Our services help these patients to maintain their oral hygiene while allowing them to remain safely in the comfort of their own home.

We can schedule just one visit while patients are recuperating or arrange a long-term arrangement. Regardless of conditions, circumstances or age of the client, in most instances our dental care team will come to the patient.

The process can be initiated several ways:

  • Social Services
  • Home Healthcare Provider
  • Family Member
  • Access Challenged Patient
  • Physician
  • Dentist
  • Health Care Manager
  • Assisted Living Facility
  • Independent Living Facility
  • Retirement Community
  • Long Term Care Facility (Skilled Nursing Home)

A personalized and private dental care appointment can be scheduled and an individualized dental care plan will be developed to meet the patient’s specific and unique oral care needs.

Patients no longer need to worry about the challenges of coordinating transportation or missing hygiene appointments.  Our team delivers quality care to patients in the convenience of their home.  Contact us to discuss the particular needs of the access challenged patient.
